​Change Program

About the Lifestyle Change Program
Starting Fall 2025, the Lifestyle Change Program offers:
  • 16 weekly in-person sessions with monthly follow-ups.
  • Support from trained lifestyle coaches.
  • Tools to improve diet, physical activity, and overall well-being.​
Fact: Losing just 5-7% of your body weight can reduce your risk of type 2 diabetes by half!

Why It Matters
Prediabetes means blood sugar levels are higher than normal, but not yet diabetes—and most people don’t know they have it. Without action, prediabetes can lead to type 2 diabetes within 5 years, increasing the risk of heart disease and stroke.
Taking simple steps—such as eating healthier, exercising more, and losing weight—can significantly reduce these risks.

Ready to improve your health? The White Earth Lifestyle Change Program is free and open to those looking to:
  • Lose weight.
  • Eat healthier.
  • Be more active.

You may have prediabetes and be at risk for Type 2 Diabetes if you: 
  • Are 45 years of age or older
  • Are overweight
  • Have a family history of type 2 diabetes
  • Are physically active fewer than 3 times per week
  • If you had diabetes while pregnant (gestational diabetes) or gave birth to a baby that weighed more than 9 pounds
Risk Test
Take the 1-minute Risk Test to see if you're at risk for prediabetes.

“So many people in our community are living with prediabetes or type 2 diabetes,” says Lisa Braun, RN for White Earth Tribal Public Health. “This program provides the tools and support to prevent or delay type 2 diabetes through healthy lifestyle changes.”
